Where are you viewing this thumbnail? In Insight or web client?
This doesn't look like
Depending on the failure, you might see errors in client or server logs.
Insight logs are in your user home directory on your client machine, E.g. /Users/wmoore/omero/log/omeroinsight.log.
You can find this in the Insight client under Help > Show Log File.
In web, you'll have to look in the server under var/log/OMEROweb.log.
Server logs themselves are in var/log/Blitz-0.log.
Do you see thumbnail failure correlating with image size in any way?
It's possible that if it's a very long thin image that in resizing to be a thumbnail (longest side 96)
that it is effectively zero pixels high/wide?
